Packets MUST include supporting documentation for each score recorded on the score
A copy of your unofficial transcript with score sheet courses highlighted plus a transfer
equivalency report (which you can get from the SCC Records office) if your letter
grade is showing “TR”.
Place all in a large envelope (do not fold any paperwork) write on the outside of
the envelope the following information as it pertains to you:
Program Name Student Name Address Phone Number Email College ID number
Packets can be dropped off in the Health Science Building Lobby beginning at 8:30
am Monday, May 10th until 4:00 pm Tuesday, May 11th (not after 8:00 pm on Monday).
You will see a large bright pink wooden box to drop packets into. Packets will be
removed promptly at 4:00 pm on Tuesday, May 11th and no additional packets or paperwork
can be submitted.
